How to Apply

Application in prescribed format (DD 40) to be submitted to Zila Sainik Welfare concerned along with:

Ø      Attested copy of discharge certificate

Ø      Death certificate of soldier

Ø      Income certificate

Ø      Relationship certificate from village officer

Ø      Certicate (Self declaration) stating non-receipt of similar benefit from any other sources
